ATOD Examination Entry Form

    By submitting this form, I am aware I am officially entering my child for their examination with the Australian Teachers of Dancing. I understand no refunds will be given and full payment will still be required if circumstances change resulting in my child not being able to sit the examination. Unless in case of injury or illness where a Doctors certificate may be required. This will then be reviewed and accepted at the discretion of the Principal and Examination Board.
